You can use A-PDF Watermark (free download here) to add a background image to pdf file with only 3 steps:

1. Click "add image watermark" icon, open the "New image Watermark" configuration window, select a background image as watermark, and then click "OK" button to create a background image watermark
2. Click "File" menu, select "Add PDF Document" item to add PDF(s) documents
3. Click "Watermark" button to add the background image watermark into pdf which you added
