You can try A-PDF Data Extractor (free download here).

The tool enables you to extract data from fix-format PDFs in batch mode:

  1. Click "Add PDF File" or "Add Folder" button to add PDF files to the list;
  2. Select a Rule for Extraction: click "Add a new rule" icon to edit a rule based on your sample file, you can save the rule and export for later use;
  3. Click "Options" to set output as single or individual Excel/ CSV/ XML, then click "Extract All" button to output file in your defined folder.
